Welcome to Fairview Nursing Plaza located at 321 Arnold Avenue Rockford, Illinois 61108. Rockford is well known to it’s neighbors as the “Forest City”. As such, Fairview Nursing Plaza is nestled in a residential area with many trees, outdoor recreational area, and safe walkways for our residents to enjoy.

Fairview Nursing Plaza is a 213 bed skilled nursing facility providing a variety of services ranging from 24-hour skilled nursing care, individualized therapies, Alzheimer’s/Dementia Care, Hospice Care, Respite Care and specialized services for the chronically mentally ill.

Fairview Nursing Plaza has two floors of resident caring. The first floor is designed to meet the needs of residents with medical conditions, including our hospital step-down Medicare wing. The second floor is primarily designed to meet the needs of residents with Alzheimer’s, Dementias, and other psychiatric diagnoses.

We treat a variety of mental illnesses in our residential housing setting, including Schizo-Affective Disorder, Major Depression, Bipolar Disorder, Schizophrenia , as well as those who are dually diagnosed with a mental illness and substance abuse.

Our psycho-social rehabilitation programs include anger management, substance abuse & med management.

We are licensed by the State of Illinois and are approved for Medicare, Medicaid, Medicaid Pending and many private insurances.
